The story picks up a couple of months after Scythe ends. Citra is doing her duties as a Scythe with Marie, and Rowan. Well, he has gone rogue and made it his mission to kill corrupt Scythes, calling himself Scythe Lucifer. A new character - Greyson Tolliver - is introduced. I have to admit I didn't realize he was a main character until the ending of the book... But for the plot, it made sense to introduced somebody who isn't... an outsider to the Scythedom. I have to admit that I missed seeing some characters in here, like Rowan or Farady, they did not have enough chapters! There were some weird parts in this story (like the whole transplant story), and definitely some sad scenes. And the first 100 pages were super slow, because there wasn't any important action in there. But let me tell you, the ending made up for it. It was phenomenal and left me shook. While writing this, I am alread about 200 pages into The Toll, and I do advice you to already have it at hand before finishing Thunderhead. Believe me, you'll want to continue asap.
